MSAF is managed/maintained by the faculty offices. Please contact your department/faculty office with this inquiry, and they should be able to assist you. 


Please note:

Only Undergrad students are eligible for the McMaster Student Absence Form (MSAF). Certificate and Graduate students are not eligible.
